Thank you for agreeing to be included in a production. ​ Please read, fill in this ONLINE Release Agreement.

RELEASE AGREEMENT

​This is a digital agreement between you and Double Farley Creative Partners ("producer") of a documentary or other creative project ("production") that you currently know as:

1 - WHAT IS A RELEASE AGREEMENT? ("TERMS")

  • You understand that when signed this form releases the producer to use what they intend to or have filmed or recorded of you ("released material") in connection with their production named above, and/or any related or subsequent productions.
  • You also understand that if you supplied, agree to supply, or had featured anything that you hold the exclusive rights for such as but not limited to personal photographs, or other creative content like music, hereby collectively known as ("Content"), and where for the avoidance of any doubt is also described below, that you grant the producers a licence to directly or indirectly copy, publish, publicly display and perform, transmit, broadcast, telecast and distribute Content worldwide and by any means now known or hereafter devised.
  • This Release Agreement and any granted Licenced Content is to be considered non-exclusive, worldwide, with perpetual rights, and is enforceable under New Zealand and applicable international law.

2 - LICENCED CONTENT (if any)

Describe any Licenced Content you own the rights to and we have requested is included in the film so should be described as part of your Release Agreement. For example a family photo you took and either showed us on camera or sent us a scan or print of. This is an optional section you can leave blank if not applicable, i.e. we just filmed or intend to film you speaking to camera.

By default we include everyone who appears in, or supplies content for a production in the credit roll at the end, on selected promotional material including an Internet Movie Database (IMDB) and often with a name caption during their initial appearance in the production.
If you don't want us to do this then you can still appear in the film but will either be uncredited or we may use a pseudonym instead. If you have specific requirements then use the box below to tell us.

Submitting your Release Agreement
